Shop Terms and Conditions
By purchasing a health package through this website, you acknowledge that you have read, understood, and agree to be bound by these Terms and Conditions. These terms constitute a binding legal agreement between you and PATIENT FIRST MEDICAL CLINIC AND DIAGNOSTICS CENTER CORPORATION ("Patient First", "we", "us", or "our").
IF YOU DO NOT AGREE TO THESE TERMS, DO NOT PURCHASE.
1. Payment & Confirmation
Service availability is guaranteed only upon: (1) Full Payment; AND (2) Confirmation of Appointment Schedule.
Full payment is required upfront. Accepted payment methods include: PayPal, Credit Card, GCash, and Bank Transfer. For GCash and Bank Transfer payments, proof of payment must be submitted to wecare@patientfirstphil.com within three (3) banking days (Monday-Friday).
• An Order Confirmation is issued upon purchase for commercial reference only. This is NOT a BIR-valid receipt or tax document.
• An official Service Invoice will be issued at the time of service redemption at the clinic, subject to BIR regulations.
2. Service Availability & Scheduling
• All services are subject to clinic availability and scheduling capacity.
• A concierge representative will contact you within 24 hours on Mondays through Saturdays, or on the next working day in case of Sundays or holidays after payment confirmation to coordinate scheduling.
• Patient First reserves the right to reschedule or cancel appointments due to operational requirements, staffing constraints, equipment issues, or any other reason. Alternative dates will be offered when available.
• Patient First is not liable for any costs, damages, or inconvenience resulting from rescheduling or cancellation.
3. Refunds, Exchanges, Transfers & Reschedulings
• Non-Refundable: All purchases are final and non-refundable.
• Exchange: Package exchanges for alternative services of equal value may be requested through wecare@patientfirstphil.com and will be considered on a case-to-case basis at the sole and absolute discretion of Patient First.
• Transfer: Packages may be transferred to another person with prior written notice to Patient First and subject to identity verification (valid government-issued ID required). The validity period is calculated from the original payment date and does not reset upon transfer. Transfer approval is at Patient First's sole discretion.
• Rescheduling: Appointment rescheduling is permitted by notifying the concierge, subject to clinic availability and scheduling capacity. Package validity period will not be extended due to rescheduling.
4. Package Validity & Forfeiture
• Validity Period: All health packages are valid for three (3) months from the date payment is received and order is confirmed by Patient First
• Strict Forfeiture: Services not redeemed, or validly exchanged, transferred, or rescheduled within the validity period will be automatically forfeited without refund, credit, or extension, without exception. Validity periods cannot be extended for any reason, including but not limited to: client scheduling conflicts, illness, travel, forgetfulness, or claimed lack of notice.
• Client Responsibility: It is the client's sole responsibility to track the validity period and schedule service redemption before expiration.
5. Client Obligations
• You must provide complete and accurate personal information. If purchasing for another person, you must provide accurate recipient information.
• You must comply with all pre-service requirements including but not limited to: fasting instructions, arrival times, preparation guidelines, and documentation requirements as communicated by the concierge.
• You must present your reference number and valid government-issued ID at the time of service redemption.
• Late arrival may result in service denial, rescheduling, or forfeiture at Patient First's discretion, without refund.
• Failure to comply with preparation requirements (e.g., fasting) may result in service denial, rescheduling, or forfeiture, without refund.
• Misrepresentation, falsification, or incomplete information may result in immediate service denial and package forfeiture without refund.
6. Limitation of Liability
• Maximum Liability: In no event shall Patient First's total liability arising from or related to a purchased package exceed the amount paid for that package.
• No Consequential Damages: Patient First shall not be liable for any indirect, incidental, special, consequential, or punitive damages including but not limited to: lost time, lost wages, travel expenses, emotional distress, or any other damages arising from delays, cancellations, rescheduling, or service issues.
• Service Provided As-Is: Services are provided on an 'as-is' basis. Patient First makes no warranties or representations regarding service quality, accuracy of results, or fitness for any particular purpose.
• Force Majeure: Patient First is not responsible for failure to perform or delays caused by circumstances beyond its reasonable control, including but not limited to: natural disasters, government actions, pandemics, power outages, internet failures, or equipment malfunctions.
• No Liability for Results: Patient First is not liable for medical test results, their interpretation, or any actions taken based on results. Results are for informational purposes only and should be discussed with a qualified healthcare provider.
7. Discounts & Insurance Coverage
• Senior Citizen & PWD Discounts: Online health packages are offered at special promotional rates. Those entitled to Senior Citizen or Persons with Disability (PWD) discounts can avail of either the promotional discount, or the discount provided under the applicable law, whichever is higher.
• HMO & Insurance: Online packages are NOT covered by any HMO, health insurance, or third-party payor. Full upfront payment is required.
• No Discount Stacking: Only one discount or promotional offer may be applied per transaction. Multiple discounts or promotional codes cannot be combined.
8. Privacy & Data Protection
• Personal information collected is protected in accordance with the Data Privacy Act of 2012 (Republic Act No. 10173).
• By completing your purchase, you consent to the collection, storage, and processing of your personal information for purposes of: service delivery, appointment coordination, results delivery, and clinic administration.
• Your information will not be shared with third parties except as required by law or as necessary for service fulfillment (e.g., laboratory partners, diagnostic service providers). By completing your purchases, you consent to the sharing of your information for the above purposes.
9. Amendments to Terms
• Patient First reserves the right to modify, amend, or update these Terms and Conditions at any time without prior notice.
• Updated terms will be posted on this website and become effective immediately upon posting.
• Continued use of services after amendments constitutes acceptance of the revised terms.
10. Governing Law & Dispute Resolution
• These Terms and Conditions shall be governed by and construed in accordance with the laws of the Republic of the Philippines.
• Any disputes arising from or related to these terms or purchased packages shall be subject to the exclusive jurisdiction of the courts of Makati City, Metro Manila, Philippines.

11. Severability
• If any provision of these Terms and Conditions is found to be invalid, illegal, or unenforceable, the remaining provisions shall remain in full force and effect.
12. Entire Agreement
• These Terms and Conditions constitute the entire agreement between you and Patient First regarding the purchase and redemption of online health packages and supersede all prior agreements, communications, or understandings, whether oral or written.
